Plans are insured by a Medicare Advantage (HMO, PPO and PFFS) organization with a Medicare contract and/or a Medicare-approved Part D sponsor. Enrollment in the plan depends on the plan’s contract renewal with Medicare.

Please bring a medication list, names of medical providers, and paper red, white, and blue Medicare cards. Formal or Community meetings = Medicare presentation and plan review on the four Augusta dates. Resource Center or Informal meetings has one-on-one presentation and review of available plans. The 17 plans from 11 a.m. - 1 pm and the Milledgeville meetings are resource or informal meetings.
